![]() Although the “Breast-Map”® System can be initiated at any time, it is reassuring to begin following a complete clinical breast exam. Complete the rist history in the bottom right corner of your “Breast-Map”® before your next clinical breast exam. Document your monthly Breast Self Exams, along with all of your women’s health exams and mammograms in the spaces provided. Take your “Breast-Map”® system with you to your annual Women’s Health exams and mammograms. During your exam, use your “Breast-Map”® as a point of reference to discuss your finding w with your health care provider. Focus on any concerns you have regarding your breast health. This is your opportunity to ask any questions. Two photocopies should be made of the bottom portion of your “Breast-Map”® at the time of your annual women’s health exam; one becomes a part of your medical chart and the other becomes part of your personal health record and is stored in your “Breast-Mapping”® tube for future reference. After the photocopies are made, the bottom portion of your “Breast-Map”® can be wiped clean with a moist tissue and is ready for another years use. Your documentation on the breast images of your “Breast-Map”® may be cleaned off or kept from year to year at your discretion. A copy of your “Breast-Mapping”® Chart Copy can be made by your physician to record his clinical exam findings. This also becomes an important part of your medical record. Finally, as you document your findings on your “Breast-Map”® you will come to understand what is normal in your breast. This same knowledge and experience will help you to recognize a change at its earliest point.
